Important Information About Your Informed Consent to Receive Electronic Tax Documents
The Purpose and Effect of Your Consent
We are required to give you “in writing” certain tax statements
related to transactions in your account. You are entitled to receive
those documents on paper. With your affirmative consent, we can provide
the documents to you electronically instead (“Electronic Tax Documents”).
The information below will help you understand the conditions and
requirements relating to the Upstart Electronic Tax Documents program.
You can always find the most up-to-date version of this
important information at https://www.upstart.com/legal_agreements.
Electronic Tax Documents specific to your account will be available
online after you log in for up to two (2) years from the
date of initial delivery.
You indicate your consent by clicking “I consent” or other button
indicating assent to, or acceptance of, the conditions and requirements
relating to the Upstart Electronic Tax Documents program, and we
record your consent. If you do not provide consent, we will send you
paper copies. Your consent will be effective until you cancel your
enrollment in the Upstart Electronic Tax Documents.
By providing consent, you confirm that you have the ability to access
the Upstart site on the web, and the ability to open, view, save, retain,
and print PDF documents as described below. This demonstrates that you
can receive the tax documents we will deliver to you. You also confirm
that you have provided a valid email address as part of the Upstart
Electronic Tax Documents enrollment process. The email address that you
provide shall be used to send notifications regarding the tax documents.

After you provide your consent, you may still request paper documents. Contact invest@upstart.com for such requests or ask questions.
To Withdraw Your Consent
You may withdraw your consent to receive tax documents electronically by emailing us at invest@upstart.com; by calling us at (650) 204-1000 during our operating hours 9:00 AM PT
to 5:00 PM PT, each Monday through Friday that are Business Days; or by writing to us via
regular mail at Upstart Network, Inc., P.O. Box 1503, San Carlos, CA 94070. If you withdraw
your consent to receive tax documents electronically, we reserve the right to charge you
additional fees for paper copies. Allow 7 business days for processing.
Electronic Notification When Upstart Electronic Tax Documents are Ready
We deliver a tax document to you by sending an email to let you know it is available. The
email will contain a link to a website where you can access the document after you login.
You acknowledge that we may amend this policy at any time by posting a revised version on
our website. If we make a material change to this policy we will also send a notice to your
registered email address.
